Many long-term conditions place a huge burden on NHS resources, as patients with conditions such as heart disease, asthma, diabetes, COPD, kidney and liver disease need frequent GP intervention, referrals to secondary care, and long-term prescription of medicine to help manage the condition.

If you are diagnosed with a long-term condition, you can make positive choices about how to manage your illness and how you live your life.  Research shows that patients who are well-informed about their condition and who are proactive in managing it do better than those who rely on their healthcare providers to spot when things are going astray, because you, the patient, are in the best position to know how you feel and when things change.
